How to Register with us
Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this). Please note that your medical records may take several weeks to arrive at the Practice.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

The Surgery receives many requests from patients either to remain with the Surgery when they move away, or for patients living some distance away to join this Practice. While we would like to please everyone and agree to these requests, we are restricted by the number of GPs and other staff physically working here. In addition patients living further away subsequently requesting home visits put these limited resources under strain.
For these reasons we will normally ask patients living out of our catchment area to find another GP more local to their homes. We will provide assistance in locating a new GP as far as we are able, but please do not ask us to recommend another GP as this is an unfair request on us (indeed we could not and would not suggest one GP is “better” than another).
We regularly receive correspondence returned advising us that a patient has “gone away” or similar. In these circumstances and where we hold no alternative address we are obliged to write to the address on our records seeking a response within 28 days. If we do not receive confirmation that the patient still resides at the address on our records, we will initiate their removal from our list.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.

Please note that if you were previously registered with a Doctor outside the UK then the Practice will not have access to your medical notes. All patients who register from abroad should contact their previous Doctor to obtain copies of their notes.
We have the right to refuse to register patients if they have a violent flag against their record.
Pre-register online
Step 1: Complete online registration form
Our online registration form will collect your details, a brief outline of your medical history and record your preferences. This information may be added to your record and will allow us to begin to provide you with medical services.
Please complete one form for each family member.
Step 2: Upload scans of photo identity and proof of address
Please deliver to the practice copies of two forms of your identity.
One form of identity must contain a photo of yourself and the other must provide evidence of your current address. You may upload your forms online.

The National Care Record Service (NCRS)
The National Care Record Service (NCRS) is the way in which health professionals now access the Summary Care Record (SCR). The SCR has not changed, it is just the way SCRs are accessed that has changed.

Organ Donation
